Our Home Care/Hospice Aides collaborate with an interdisciplinary team of social workers, RN case managers, caregivers, chaplains, and volunteers to enhance the lives of our patients and their families.
Compensation: $21.00-$28.00 (DOE).
Sign-on Bonus: $1,000, paid after 90 days of employment.
Shift: Monday-Friday, flexible full-time hours

What You Get To Do:
- Assist the patient with all aspects of personal care, ambulation, and positioning according to the plan of care.
- Ensure patients and caregivers have all necessary supplies to provide personal care.
- Help guide the family/caregiver regarding the patient's personal care.
- Create professional relationships with the patient, family, and caregivers while maintaining the confidentiality of the patient and related health information.
- Assess the patient's ability to tolerate treatment before beginning care. Assess patient condition aftercare.
- Perform delegated tasks from the RN to provide some skilled portions of the patient's care.
- Report any changes or concerns with RN case managers to ensure that care plans are updated and accurate.
- Opportunities to assist RNs directly during patient care for skills requiring two people.
- Meet the patient's and self's safety needs by utilizing good body mechanics and proper use of equipment, including mechanical lifts.
Required Skills & Qualifications:
- Valid CNA License.
- Ability to pass a background test, drug screen, and TB test.
- Graduate of an accredited nursing school.
- One to two years of recent medical care experience in an institutional setting is preferred. Ideal candidates will possess 1-2 years of home health or hospice experience.
- Current automobile insurance is required. A valid Oregon driver's license and reliable transportation.
- Valid CPR Certification.
- Strong communication and interpersonal skills.
- Ability to work independently and collaboratively within an interdisciplinary team.
