Compensation: $40.00-$55.63 (DOE).
Location: Bend, Oregon.
Schedule: Full-time, 5 or 4 days per week available.

- Assists in strategic planning, including identifying opportunities for additional or improved services to meet customer needs.
- Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Partners in Care.
- Assists with the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy. Does not direct but follows the implementation.
- Completes referral worksheets for all services and requests all needed information from referral sources.
- Faxes requests for CTI to physician and obtains physician signature.
- Enters ROC order in Electronic Medical Record (EMR).
- Assists with incoming calls and takes referrals.
- Notifies Nurse Practitioner and MD team if a hospice patient is in the 3rd or greater benefit period needing pre-admit F2F.
- Establishes and maintains positive working relationships with current and potential referral sources.
- Ensures compliance with all state, federal, and referral intake regulatory requirements.
- Checks Medicare insurance for Hospice referrals when administrative staff is not available. If patient is in the second or greater benefit period, enters information into electronic medical record (EMR) to generate correct certification date.
- Checks Medicaid eligibility for hospice.
- Adheres to all policies, procedures and regulations regarding safe ethical conduct, security, and risk management.
Required Skills & Qualifications:
- Graduate of an accredited school of nursing with current licensure to practice professional nursing in the State of Oregon.
- At least two years’ experience in health care.
- Excellent assessment, problem-solving skills, nursing skills per competency checklist.
- Strong clinical judgment and patient triaging skills.
- Efficient time management with minimal oversight.
- Proficiency with Adobe Acrobat, Microsoft Word, Microsoft Outlook.
- Ability to learn and effectively utilize a large array of different computer programs and platforms.
- Ability to navigate between multiple software systems and programs concurrently, often while on the phone.
- Positive phone interaction and customer service.
- Ability to pass a background test, drug screen, and TB test.
- Ideal candidates will possess 1-2 years of home health or hospice experience.
- Valid CPR Certification.
- Current automobile insurance, valid Oregon driver’s license, and reliable transportation required for those working in the field.
- Ability to work independently and collaboratively within an interdisciplinary team.
