What You Get To Do: The Transitions Coordinator implements and builds community awareness of the Transitions Program for the entire service area for Partners In Care. Provides nonclinical case management and community resource navigation. Makes referrals to appropriate community services as needed by the client/family. Interacts with administration, volunteer department, field clinicians and supervisors, outreach team, volunteers, physicians, community agencies and the public.
- Effectively case manages Transitions client services.
- Establishes and maintains positive working relationships with current and potential Transition clients/referral sources.
- Communicates with providers regarding their patients (our clients) on Transitions, as appropriate.
- Effectively coordinates community services as client/family needs indicate.
- Public speaking regarding the Transitions program to community.
- Outreach responsibilities as determined by the department, supervisor and organization.
- Works with Volunteer department to implement volunteer services, when appropriate, for Transitions clients.
- Maintains all Transitions program policies and procedures; and HIPPA privacy practices.
- Attends weekly meetings with Transitions team, quarterly meetings with the palliative care team and monthly meetings with the outreach department.
- Maintains accurate documentation/records per organization and department standards.
- Informs Transitions team members about new clients and updates.
- Participates in phone coverage rotation schedule.
- Adheres to the PIC mission and values.
- Supports the organizational mission.
Required Skills and Qualifications:
- Bachelor’s degree or equivalent experience.
- CHW certification and Spanish speaking preferred, but not required.
- One year of home health/ hospice care experience preferred.
- Demonstrates excellent observation, verbal, and written communication skills.
- Adequate verbal, written, and organizational skills needed to perform job functions.
- Case management experience preferred.
- Knowledge and understanding of support resources in Central Oregon preferred.
- Local/regional travel by personal vehicle during the working day, centralized from the Bend office and extending throughout Central Oregon.
- Ability to pass a background test, drug screen, and TB test.
- Current automobile insurance, valid Oregon driver’s license, and reliable transportation required for those working in the field.
- Strong communication and interpersonal skills.
- Ability to work independently and collaboratively within an interdisciplinary team.

Compensation: $22.00 - $36.13 per hour
